I’m Lily, the founder of Liora Ministry™. I started Liora because I have seen how easily important details can get missed when patients move between appointments, hospital teams, discharge, and home.

In our session, I will take time to understand what matters most to you and your family right now — your priorities, your goals, your worries, and what may be getting in the way. From there, we can review the information you bring, prepare questions, discuss what may need follow-up, and identify practical next steps.

My nursing background includes caring for patients who needed close monitoring after surgery, supporting cancer and palliative care teams, and teaching nurses who care for patients recovering in hospital, waiting in hospital for the next place of care (Alternate Level of Care, or ALC), or needing longer-term medical support. I have also supported patients living with delirium, dementia, and complex health needs, and worked with long-term care homes, emergency departments, and hospital medicine teams.

My style is detailed, practical, and focused on communication. I pay close attention to small details because they can affect how families understand the plan, ask questions, and follow through.
